Clinical Usefulness of Duplex Ultrasonography in the Epidemic Hemorrhagic Fever

PURPOSE : Epidemic hemorrhagic fever is often a fatal disease and manifested by acute renal failure. Theauthor performed gray-scale and Doppler ultrasonography of the kidneys in serologically confirmed 14 patients withthis disease on their diverse clinical stages to verify the clinical usefulness of this duplex ultrasonography.MATERIAL & METHODS : A total of 72 examinations were performed (mean, 5 times per patient). On gray-scalesonography, maximal pole-to-pole lengths and cross-sectional areas of both kidneys were measured, and on Dopplersonography, mean resistive indices (RI) were calculated after six analyses of doppler spectrums from theinterlobar arteries of both kidneys. These data were correlated with serum BUN, creatinine and the amount of 24hour urine output on the same day. RESULTS :RI was statistically well correlated with serum BUN, creatinine,amount of 24 hour urine output and maximal cross-sectional areas (p < 0.001). RI showed earlier declining changesas the clinical stage progressed and was followed by the changes of other parameters. CONCLUSION : The authorpresumes that duplex ultrasonography is a rapid, easy and clinically useful method to evaluate current injury andrecovery of renal functions in the patients with epidemic hemorrhagic fever.
