Histologic Correlatron of GastrointestinaI UItrasound Images

Endoscopic or abdominal/transrectal ultrasonography enables high resolution imaging of the diagnosis ofgastrointestinal diseases. However, histologic correlation of the findings at gastrointestinal ultrasound imageshas not been well established. We performed ultrasonography of 70 surgically rescted gastrointestinal specimens byusing 7.5MHz linear array probes, and compared the ultrasound images with corresponding histologic sections. Thefive layers seen on ultrasound images of the normal intestinal wall consisted of (1) the acoustic interfacebetween the fluid bathing the surgical specimen and the mucosal surface, and some portions of the superficialmucosa, (2) the deep mucosa including the muscularis mucosae and the remaining portions of the superficial mucosa,(3) the submucosa plus the acoustic interface between the submucosa and the muscularis propria, (4) the muscularispropria minus the acoustic interface between the submucosa and the muscularis propria(cf, inner echogenic linecorresponding to the fibrous tissue between circular and longitudinal muscles in colon), and (5) the serosa andthe subserosal fat plus the acoustic interface between the serosa and peri-enteric tissue or the fluid bathing thesurgical specimen. When properly interpreted on the basis of this histologic correlation, ultrasound images of thegastrointestinal wall can provide useful diagnostic information.
