Detection of appendicoliths in patients with acute appendicitis: comparative study with US and plainradiography)

The sensitivities of graded compression sonography (US) and plain radiography for detecting appendicoliths in21 patients with surgically confirmed acute appendicitis and appendicoliths wre retrospectively evaluated. 27appendicoliths were identified in the resected specimens of 21 patients. The surgical and pathologic correlationon a lesion-by -lesion basis to allow determination of the sensitivity of each imaging test was obtained. Thesensitivities were 81% (22 of 27 appendicoliths) for plain radiography. The difference between the sensitivity ofUS and plain radiography was statistically significant (p<.05). Of the appendicoliths smaller than 10mm inmaximldiameter, US depicted 74% (14 of 19 appendicoliths) and plain radiography 37% (7 of 19 appendicoliths). Weconclude that for radiologic detection of appendicoliths, sonography is more sensitive procedure than plainradiography.
