(Preoperative staging of rectal cancer: comparison of TES with CT)

Transrectal endosonography (TES) for preoperative staging was performed to assess the depth of wall invasion,perirectal fat infiltration,and perirectal lymph node metastasis in 15 patients with primary rectal carcinoma.Nene of these patients underwent both TES and computed tomography (CT) for preoperative staging. The resultsconcerning TES and CT were prospectively compared and then correlated with histopathologic findings according toAstler-Coller modification of the Duke's classification. The sensitivity of perirectal fat infiltration on TES andCT was 87.5% and 50% respectively, and specificity was 100% and 33.3% respectively. The sensitivity of perirectallymph node metastasis on TES and CT was 83.3% and 75%, and specificity was 77.7% and 60% respectively. overallaccuracy of staging with TES and CT in rectal carcinoma was 67% and 22% respectively. TES overstaged in 20% andunderstaged in 13%, and CT overstaged in 45% and understaged in 33%. As our results suggest that TES is superiorto CT in the detection of depth of wall invasion, perirectal fat infiltration and perirectal lymph nodemetastasis, for preoperative staging of rectal carcinoma.
