(Dynamic contrast enhanced ultrasonography with carbon dioxide microbubbles in primary liver cancer)

Dynamic contrast enhanced ultrasonography (enhanced US) with intraarterial infusion of carbon dioxidemicrobubbles was performed in 23 patients with 30 nodules of primary liver cancer (22 cases of hepatocellularcarcinoma and one case of cholangiocarcinoma). 17 nodules were surgically confirmed and 13 nodules wereradiologically confirmed on Lipiodol CT or MR imaging. All nodules of HCC were less than 5cm in diameter. Weevaluated the echogenic pattern of primary liver cancer on enhanced US and compared the detectability of the tumornodules on enhanced US with that of plain US, CT, angiography, Lipiodol-US and Lipiodol-CT. 90% of small HCCshowed hyperechoic pattern and 10% showed isoechoic pattern on enhanced US. Characteristic dynamic change of thesmall HCC on enhanced US was hyperechogenicity arising from the periphery of the mass at early arterial phase anddense hyperechoic mass at venous phase. One case of the cholangiocarcinoma showed hypoechoic mass with peripheralenhancement. Number of detected tumor nodules was well correlated between enhanced US and Lipiodol CT (or surgicalspecimen) in 16 cases. Detection rates of tumor nodules was 73% on plain US, 77% on CT, 70% on agiography and 86%on enhanced US and 81% on Lipiodol-US. The newly developed enhanced US technique, which is easy to perform duringhepatic angiographic procedure, would be useful in assessing the nature of the primary liver cancer and detectingthe small HCC nodules.
