Usefulness of intraoperative sonography in the management of hepatocellular carcinoma)

Intraoperative sonography was performed on 74 tumor nodules in 61 patients with hepatocellular carcinoma to evaluate the detectability of the tumors as compared with conventional sonography. CT, angiography and Lipiodol-CT. Detection rates of tumor nodules were 84% on sonography, 90% on CT scans, 89% on angiography, 100% on Lipiodol-CT and intraoperative sonography. In operating field, tumors were invisible in 27 of 74 cases (36%) and nonpalpable in 20 of 74 cases (27%). In 30 tumors less than 2 cm in diameter, invisible tumors were 56% and nonpalpable tumors were 53% Our results suggest that intraoperative sonography is the final diagnostin imaging procedure before surgery and in cases of invisible and nonpalpable tumors in operating field, this procedure is mandatory to improve surgical results.
