Preoperative Vascular Mapping for Hemodialysis:The Role of Postoperative Patency

PURPOSE : To assess the (effectiveness?) of preoperative vascular mapping in predicting the postoperative patency of AVF or AVG. MATERIALS and METHODS : 66 patients who underwent hemodialysis for end-stage renal failure were observed prospectively from January 2001 till April 2003. The postoperative patency rate was compared between the control group and the mapping group who underwent preoperative vascular mapping. All 66 patients were classified into either subgroup F (AVF) or subgroup G (AVG) . Statistical analysis was done using Fisher`s Exact test and the Chi-square test. RESULTS : The mapping group was composed of 34 patients, of whom 27 patients were classified as subgroup F and 7 as subgroup G. These 34 patients were composed of 14 men and 20 women, aged between 34 and 86 years (mean, 57.4). 2 patients were reoperated in each subgroup (F (7.4%) and G (28.6%)). The control group was composed of 32 patients, of whom 27 patients were classified as subgroup F and five as subgroup G. These 32 patients were composed of 20 men and 12 women, aged between 31 and 84 years (mean, 60.3). In the control group, there were 5 reoperative cases in subgroup F (25.9%) and 2 in subgroup G (40.0%). There was a statistically significant trend in the F subgroup between the mapping group and the control group (p=0.068). CONCLUSION : Sonographic vascular mapping is a useful technique for the preoperative evaluation of AVF.
